Effect of laparoscopic fundoplication treatment on gastroesophageal reflux disease-related respiratory symptoms

Abstract: Gastroesophageal reflux disease (GERD) was diagnosed and fundoplication was performed, which relieved the symptoms. This paper explores the effect of fundoplication on GERD-related respiratory symptoms. From January 2007 to October 2008, 64 patients with respiratory symptoms related to GERD were consecutively enrolled for laparoscopic fundoplication. Of these, 60 patients had respiratory symptoms mainly combined with gastroesophageal reflux, while four had respiratory symptoms only. All patients had GERD confirmed, and medical treatment provided before surgery resulted in an insufficiently favorable effect or even producing further complications. All the patients were requested to fill a questionnaire form regarding the severity of symptoms and their medication so as to make an evaluation score for the follow-up. The outcome of respiratory symptoms after surgery was as follows: excellent in 23 cases (35.9%); good in 28 (43.8%); fair in 5 (7.8%); poor in 8 (12.5%). The mean respiratory symptom score decreased from 6.3±2.65 to 2.33±2.37. Laparoscopic fundoplication can be an effective treatment for the majority of patients with GERD-related respiratory symptoms.

Detection of maternal serum thromboxane B2 in different durations of pregnancy and prediction of preeclampsia

Abstract: This study was aimed to detect maternal serum levels of thromboxane B2 (TXB2) in different durations of pregnancy and explore the predictive value of TXB2 for preeclampsia. By employing a prospective and double-blind study method, 180 pregnant women with previously normal blood pressures during their progestation were included in the study. Peripheral venous blood samples were obtained during 10–14 (period I), 20–24 (period II) and 30–34 (period III) weeks of gestation. Maternal serum levels of TXB2 were measured by enzyme-linked immunoassay. The pathogenetic condition and pregnancy outcomes of these cases were observed. (1) Among the 180 previously normotensive women, ten developed preeclampsia (case group; four severe and six mild preeclampsia). One hundred and seventy remained normal till the end of pregnancy (control group). (2) With development of pregnancy, the levels of maternal serum TXB2 in the 180 cases gradually ascended as seen through the Wilcoxon Signed Ranks Test. There was a statistical significance in maternal TXB2 levels between two different gestational stages (<0.01). (3) The levels of maternal serum TXB2 were slightly higher in the case group than in the control group during period I of gestation, but the difference was not statistically significant (>0.05); the levels of TXB2 were significantly higher in the case group than in the control group during periods II and III of gestation (both <0.05). (3) The best cutoff points of maternal TXB2 were 3750 and 4400ng/mL during periods II and III of gestation by receiver operator characteristic (ROC) curve; and the sensitivity, specificity, positive predictive value, negative predictive value and odds radio of TXB2 in predicting preeclampsia were 80%, 69.5%, 13.3%, 98.33%, 9.11 and 90%, 68.82%, 13.84%, 99.13, 19.86, respectively. Higher levels of maternal serum TXB2 were detected a long time before clinical symptoms appeared. The maternal serum TXB2 after 20 weeks of gestation had predictive value, and the index after 30 weeks was superior to that prior to 30 weeks .

Abstract: p38 regulated/activated protein kinase (PRAK) plays a key role in cell senescence and tumor suppression. The aim of this study was to investigate if PRAK had effect on cell proliferation. The growth of and mouse embryonic fibroblast (MEF) cells was measured by methylthiazoletetrazolium (MTT) colorimetric assay, and the proportion of the cell number in different phases of the cell cycle was analyzed by flow cytometry. The growth curves showed that the growth rate was notably decreased, and cell double time was elongated in cells; moreover, the number of cells was decreased by 44.5% compared with that of cells cultured for 96h, suggesting that G/M transition is inhibited in cells. Meanwhile, G/S transition was also inhibited in cells, observed with flow cytometry analysis. The ratios of G/G, G/M, and S phases of cells were 44.9%, 12.2%, and 42.9%, respectively, while those of cells were 55.3%, 7.3%, and 37.4%, respectively. There were 23.1% increase and 12.7% decrease of the number of cells in G and S phases comparison with that of cells, respectively. Taken together, gene knockout in MEF cells leads to cell cycle arrest and proliferation inhibition.

Successful infrainguinal revascularization with autologous vein for inadvertent stripping of superficial femoral artery: a case report

Abstract: A referral patient who had previously undergone varicose vein surgery was admitted as an emergency case. On admission, the patient complained of intolerable pain, paralysis and paresthesia of the affected limb, which was characterized by acute arterial ischemia symptoms. Color Doppler of the artery of the affected limb indicated that no blood flow signal existed in the superficial femoral artery. During exploratory operation, we found that the right superficial femoral artery instead of the great saphenous vein of the affected limb had been stripped and ligated. Therefore, the intact right great saphenous vein was taken for auto-transplantation by inverse end-to-end anastomosis to the proximal and distal residual superficial femoral artery, which resulted in gradual recovery. Except for ischemic reperfusion injury, no other post-operative complications occurred after a 10 month follow-up; however, the long-term curative effect needs further observation. Here we report our treatment experience.

Data mining of microarray for differentially expressed genes in liver metastasis from gastric cancer

Abstract: Tumor metastasis is the leading cause of death for gastric cancer. Metastasis is the main reason for the failure of clinical treatment for gastric cancer. In order to find metastasis-related genes and abnormal signal transduction pathway of high-invasive gastric cancer, samples of gastric cancer with liver metastasis were collected for microarray detection; up-regulated or down-regulated genes in all three cases were simultaneously screened out. Subsequently, from the preliminary screened genes, molecular pathways possibly impacting liver metastasis from gastric cancer were investigated by the Gene Cluster with Literature Profiles (GenCLip) analysis software. Many biological effects including apoptosis have been validated. Functional analysis of differentially expressed genes revealed that a variety of biological pathways, such as blood circulation and gas exchange, vasodilation and vasoconstriction regulation, and immune defense, could be significantly activated. Besides, gene sequences, specific keywords or gene regulatory networks were further searched by GenCLiP. We conclude that data mining allows to quickly identify a series of special signal transduction pathways involving abnormally expressed genes.

Effect of atorvastatin on tumor growth and metastasis in a breast cancer cell xenograft model and its mechanism

Abstract: This paper aims to evaluate the effects and the possible mechanisms of atorvastatin on tumor growth and metastasis in a xenograft tumor model. Twenty-four female athymic BALB/C mice with MDA-MB-435 xenograft tumors were randomly assigned to three groups: a control group, a low-dose atorvastatin treatment group, and a high-dose atorvastatin treatment group. The mice in the treatment groups began to be administered with atorvastatin (10 or 20 mg/kg per day) when the xenograft tumors reached 1 cm in diameter. At the end of the experiment, the tumor volume and weight and the lung metastasis colonies of each mouse were measured. Western blotting was applied to detect phosphorylation of protein kinase B (PKB, Akt), extracellular signal regulated kinase (ERK), c-Jun N-terminal Kinase (JNK), and the expression of cytochrome P450 (CYP) subtype CYP2J2. Atorvastatin suppressed xenograft tumor growth and metastasis both in the low-dose and the high-dose treatment groups ( < 0.05). Atorvastatin also decreased the phosphorylated Akt (p-Akt) and p-ERK but increased p-JNK expression. However, atorvastatin did not alter the expression of CYP2J2 in tumor tissue. This suggests that atorvastatin has the efficacy of suppressing tumor growth and metastasis . These effects were not dependent on down-regulation of CYP2J2 expression.

Effect of pEGFP-survivin on GBC-SD cell growth and chemotherapy sensitivity

Abstract: This paper is aimed to investigate the effect of survivin shRNA on chemotherapy resistance in human gallbladder carcinoma GBC-SD cells. The viability of human gallbladder carcinoma GBC-SD, GBC-SD/enhanced green fluorescent protein (EGFP), and GBC-SD/survivin cells was detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, and mRNA and protein of survivin were tested by reverse transcription-polymerase chain reaction (RT-PCR) and Western blot. After the cells were treated with cisplatin (DDP) (3.0 μg/mL) for the same time, cell survival rate and IC was detected with MTT, cell apoptosis was detected with fluorescence-activated cell sorting (FACS), and the nuclear alteration was observed by TdT-mediated deoxyuridine triphosphate nick end labeling (TUNEL). In addition, caspase-3 activity was detected by using colorimetric method. Cell viability was decreased significantly in GBC-SD/survivin cells, and survivin expression was decreased significantly (mRNA and protein of survivin were decreased by 74.7% and 71.5%, respectively). After treatment with DDP, cell survival rate and IC was decreased significantly (2.03±0.24 μg/mL) in GBC-SD/survivin cells, while apoptotic rate (84.3%) was elevated significantly as compared with the other two groups. There were brown apoptotic nuclei in all the cells. Caspase-3 activity in all the cells was increased at first and then decreased, but the caspase-3 activity in GBC-SD/survivin cells was significantly higher than the other two groups. The survivin shRNA could down-regulate the expression of survivin in GBC-SD cells significantly and improve the sensibility to chemotherapy.

Gene and protein expression of proteinase-activated receptor-1, 2 in a murine model of acute graft host disease

Abstract: Proteinase-activated receptors (PARs) are a novel subclass of seven transmembrane-spanning, G protein-coupled receptors. PAR-1 and PAR-2 are widely expressed in a variety of cells and are found to be involved in many physiological and pathological processes including inflammation and immune response. However, little is known about the function of PAR-1, 2 in acute graft host disease (GVHD). In the present study, we first detected the expression of PAR-1, 2 protein and mRNA in a murine model of acute GVHD using the methods of immunohistochemistry, Western blot and quantitative real-time polymerase chain reaction (PCR). Syngeneic hematopoietic stem cell transplantation (HSCT) mice served as controls. The relative gene expression level of PAR-1 was significantly increased in the skin, liver, small intestine of allogeneic HSCT mice (in skin: 0.039±0.013 0.008±0.002 of controls, =0.009; in liver: 0.165±0.006 0.017±0.006 of controls, =0.004; in small intestine: 0.215±0.009 0.016±0.002 of controls, =0.003), but not in the stomach, lung and kidney of allogeneic HSCT mice (>0.05). PAR-2 mRNA expression in the liver and small intestine of allogeneic HSCT mice (in liver: 0.010±0.002 0.003±0.001 of controls, =0.008; in small intestine: 0.006±0.001 0.003±0.001 of controls, =0.024) was increased significantly, but PAR-2 mRNA expression in the other organs (>0.05) was not found to be significantly elevated. PAR-1, 2 protein expression was in accordance with the mRNA expression, as shown by Western blot. Using immunohistochemistry the present study demonstrated that there was strong PAR-1, 2 immunoreactivity in the epithelial cell and vascular endothelial cell of target organs of acute GVHD. Our findings of markedly increased expression of PAR-1, 2 in target organs of acute GVHD suggest that PAR-1 and PAR-2 may play an important role in the pathogenesis of acute GVHD.

Expression of PC-cell-derived growth factor in breast cancer

Abstract: This study is mainly aimed at evaluating the expression of PC-cell-derived growth factor (PCDGF) in breast cancer and breast adenofibroma, and to compare with other commonly used clinical pathological indices, then to investigate the diagnostic and targeted therapeutic purpose of PCDGF in breast cancer tissue. In this study, we detected the expression of PCDGF, p53 and CerbB-2 in breast cancer tissue and the expression of PCDGF in breast adenofibroma tissue by immunohistochemical method, and analyzed the relationship between them. We found that PCDGF was expressed in most breast cancer tissue, but was not in breast adenofibroma tissue, and the expression of PCDGF was related with the tumor’s pathological category and the expression of estrogen receptor (ER) and progesterone receptor (PR) and p53, but there was no statistical dependability between PCDGF and cerbB-2. From this study, we predict that PCDGF may serve as a marker in the secondary diagnosis of breast cancer, and may participate in the generation and differentiation of breast cancer cells, and become an effective target of therapy for breast cancer.

Cyclooxygenase-2 gene-1195G/A genotype is associated with the risk of HBV-induced HCC: A case-control study in Han Chinese people

Abstract: This study aimed to identify functional single nucleotide polymorphisms in the cyclooxygenase-2 gene promoter and evaluate their effects on the risk of primary hepatocellular carcinoma (HCC) with hepatitis B virus (HBV) infection. We conducted a population-based, case-control study enrolling 630 Han Chinese people in Hubei province. Subjects included primary HCC patients with HBV infection (=210), chronic hepatitis B cases (=210) and healthy Han Chinese (=210). -1195G/A polymorphism was analyzed using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and sequencing analysis. We found-1195A allele carriers had a higher risk of HCC with HBV infection (OR, 0.72; 95% CI, 0.548–0.946). The-1195A allele might be used as a marker in screening individuals at high risk of HCC with HBV infection.

Laparoscopic sigmoid vaginoplasty in women with Mayer-Rokitansky-Kuster-Hauser syndrome

Abstract: The feasibility and clinical therapeutic effects of laparoscopic sigmoid vaginoplasty in women with Mayer-Rokitansky-Kuster-Hauser syndrome (MRKHs) were explored. The records of 11 MRKHs patients who underwent laparoscopic sigmoid vaginoplasty from 2003 to 2005 were reviewed, and long-term results were evaluated by follow-up. The mean operating time was 234min (range, 130―300min), the mean hospital stay was 9.4 days (range, 7―15 days), and the mean hemoglobin drop was 1.91g/dL (range, 1.6―3.2g/dL). A functional neovagina was created measuring 11 to 14cm in length and two fingers in breadth in all patients. No introitus stenosis was observed. No intra-operative or postoperative bowel complication occurred. At the 3rd postoperative month, the first intercourse began. One patient was lost to follow-up. One had no intercourse and was required to wear vaginal mold occasionally. None of the other nine women (100%) complained of local irritation or dyspareunia. They were satisfied with their sexual life. The cosmetic results were excellent. The laparoscopic sigmoid vaginoplasty realizes to make a functional neovagina. The main advantage is its minimal invasiveness. It is an ideal procedure for MRKHs patients.

Relationship between the incidence of HFRS and changes of land-use in Big Three Gorges area of Chongqing Municipality, China

Abstract: After the establishment of big reservoirs, the environment along the Yangtze River, China, has changed. The alterations may influence people’s health. This study intended to find out the relationships between the environmental changes and the incidence of hemorrhagic fever with renal syndrome (HFRS) in the Big Three Gorges Area (BTGA) of Chongqing Municipality, China. Land-use thematic maps were applied in the research. The results show that there was a significant relationship between the land-use change in the grass area and the cumulative HFRS incidence (correlation coefficient= − 0.676, = 0.011). In the BTGA, the land was submerged because water filled in the reservoir, and meanwhile, the government called for returning land for farming to forestry. It is concluded that the changes of environment may influence the incidence of HFRS.

Review of recent guidelines for the management of severe sepsis and septic shock

Abstract: Severe sepsis and septic shock affect millions of patients and are major causes of mortality worldwide. Advancements in treatment and disease management led to a decline in in-hospital mortality from 27.8% (1979–1984) to 17.9% (1995 to 2000). In this article, we systemically review recent guidelines for the management of severe sepsis and septic shock published in 2008 by the International Surviving Sepsis Campaign Guidelines Committee. The 2008 Surviving Sepsis guidelines recommend protocolized resuscitation with goals to maintain central venous pressure≥8–12mmHg, mean arterial pressure≥65mmHg, urine output≥0.5mL·kg·h and central venous oxygen saturation≥70% (or mixed venous≥65%). Further fluid administration, transfusion of packed red blood cells to achieve a hematocrit of≥30% and/or infusion of dobutamine max 20µg·kg·min are advised if venous O saturations remain below 70%. In patients with decreased ventricular compliance or mechanical ventilation, a target central venous pressure of 12–15mmHg is recommended. Intravenous antibiotic administration within the first hour of recognizing severe sepsis and septic shock is essential, while use of corticosteroids in sepsis is controversial. The mechanisms by which activated protein C improves clinical outcomes in sepsis are unknown. Therapy with activated protein C is approved for patients with severe sepsis and an increased risk of death [Acute Physiology and Chronic Health Evaluation II (APACHE II)>25]. Bicarbonate therapy is discouraged. Intravenous insulin should be used to control hyperglycemia in patients with severe sepsis following stabilization in the intensive care unit.
