摘要

Abstract

One Gram-negative bacillus was isolated from a brain sample of a pig with neurological symptoms. Pathological examination showed meningitis at necropsy. Ochrobactrum anthropi (O. anthropi) was successfully isolated from the brain sample and was confirmed by biochemical reaction results (API 20 NE) and gene sequencing. The strain was highly resistant to beta-lactam antibiotics. Mice were experimentally infected with O. anthropi and showed typical meningitis. This is the first report on O. anthropi isolated from a pig, and indicates that O. anthropi may have a broader host spectrum of infection.

1. 引言

人苍白杆菌（O. anthropi）是一种在环境中广泛存在的革兰氏阴性杆菌。它最先被归类于CDC Vd群[1]，属于布鲁氏菌科[2]。O. anthropi 曾被认为是一种人类条件致病菌，主要感染免疫功能低下的患者[1,3,4]。然而，一些报道表明O. anthropi 也可以引起健康人的感染[5–7]。O. anthropi 感染的表现包含眼内炎[6]、心内膜炎、脑膜炎、骨软骨炎、骨髓炎、胰腺脓肿、穿刺伤口、骨盆脓肿和尿路感染[8–14]。据报道，除感染人类以外，O. anthropi 还可以感染其他哺乳动物。Franci等[15]首次报道了健康犬被O. anthropi 感染的报告，该犬在常规牙齿清洁后被感染，原因是麻醉液被O. anthropi 污染。ElAdawy等[16]从火鸡的盲肠中分离出O. anthropi 。迄今为止，尚未有从其他物种中分离到O. anthropi 的报道。在本研究中，我们首次从猪体内分离并鉴定了一株O. anthropi 。

2. 材料与方法

2.1. 猪临床样本中细菌的分离

猪脑样本来自于中国江西省的一家商业化的养猪场。该猪有神经症状，剖检观察显示有脑膜炎。对该猪脑样本进行组织病理学检查和细菌分离。在无菌条件下用棉拭子从脑样本中取样以避免污染。将分离物在含有10%新生牛血清（浙江天杭生物技术有限公司，中国）的胰蛋白胨大豆琼脂（TSA; BD DifcoTM ，美国BD公司）上于37 ℃培养24 h。革兰氏染色根据Gerhardt等[17]描述的方法进行。使用API 20 NE（法国生物梅里埃公司）对分离菌进行生化鉴定，方法参照厂家的说明书。

2.2. DNA 模板的制备

使用金唯智公司（中国）的细菌基因组DNA试剂盒，对24 h的细菌培养物进行DNA模板提取，方法参照厂家的说明书。

2.3. PCR 扩增 16S rRNA 和 recA 基因及测序分析和系统进化树的构建

参 照Scholz等[18]描述的方法对细菌16S rRNA基因和recA 基 因 进 行 扩 增。 正 向 引 物27f（5′-AGAGTTTGATCMTGGCTCAG-3′）和反向引物1492r（5′-AAGTCGTAACAAGGT ARCCG-3′）用于扩增16S rRNA基因，扩增的目的片段大小约1400 bp。聚合酶链反应（PCR）程序如下：94 ℃ 5min；随后94 ℃ 60 s，56 ℃ 30 s，72 ℃ 90 s，30个 循 环； 最 后72 ℃ 10 min。PCR产物使用1%琼脂糖凝胶进行电泳分析。recA 基因扩增引物为recA-f (5′-ATGTCTCAAAAAAA TTCATTGCGAC-3′)和recA-r (5′-AGCATCTTCTTCCG GTCCGC-3′)；扩增recA基因条带大小为1065 bp。PCR程 序 如 下：94 ℃ 5 min；随 后94 ℃ 30 s，58 ℃ 30 s，72 ℃ 60 s，30个循环；最后72 ℃ 10 min。PCR产物使用1%琼脂糖凝胶进行电泳分析。

将PCR产物送至金唯智公司进行测序。序列在美国国家生物技术信息中心（NCBI）数据库中使用BLASTnx进行了BLASTed处理（其中，BLAST为基本局部比对搜索工具）。使用CLUSTAL X [19]来比对序列。系统进化树基于16S rRNA基因（rrs）和recA 基因，使用软件MEGA版本5 [21,22]采用Kimura的双参数模型进行分析[20]。进化树枝干上的数字表示bootstrap test重复测试（1000次重复）时相关物种成簇的树的比例[23]。

2.4. 基因组测序和 BLAST 分析

基因组序列在NCBI数据库中通过基因组BLAST工具进行BLAST分析。苍白杆菌属（Ochrobactrum sp.）和布鲁氏菌属（Brucella sp.）的recA、16S rRNA基因的部分序列的GenBank登录号，以及部分苍白杆菌（Ochrobactrum）的全基因组序列的GenBank登录号见Appendix A中的Table S1。

2.5. 药敏试验

参照Bauer等[ 24 ]描述的纸片扩散法对分离株进行抗生素敏感性试验。将分离菌株接种于含10%新生牛血清的TSA平板，37 ℃培养24 h。将培养物用磷酸盐缓冲液（PBS, 0.01 mol·L–1 , pH 7.2）制备成菌悬液，浓度为1× 108 CFU·mL–1 （其中，CFU代表菌落形成单位）。用无菌棉签沾取菌悬液涂布于含有10%新生牛血清的TSA平板上，随后将药敏纸片（杭州微生物试剂有限公司）放在接种后的琼脂表面上。将平板底部朝上置于35 ℃孵育24 h，测量抑菌圈直径。参照动物细菌纸片和稀释法药敏试验判定标准发布的参数进行结果判定[25]。

2.6. 小鼠感染试验

将分离株接种含有10%新生牛血清的TSA平板，37 ℃下培养24 h，用PBS（0.01 mol·L–1 , pH 7.2）制成活菌含量为5×109 ~2×1010 CFU·mL–1 的系列菌悬液。通过腹腔注射感染BALB/c小鼠，每只小鼠注射菌液0.2 mL，剂量为每只小鼠1×109 ~4×109 CFU。对死亡小鼠脑组织样本进行组织病理学观察。该动物试验参照国家兽用药品工程技术研究中心动物保健和伦理委员会批准的规定进行。

2.7. 组织病理学观察

将脑样品用10%福尔马林溶液固定过夜，然后包埋在石蜡中。将脑组织切片并用苏木精和伊红染色以进行组织病理学评估。

3. 结果

3.1. 组织病理学检查和细菌分离鉴定

猪脑样本的组织病理学结果显示脑白质有坏死性结节、灰质神经元坏死和大量中性粒细胞浸润，如图1所示。将脑样本划线TSA平板分离到纯的光滑小菌落。革兰氏染色为阴性杆菌（图2）。

该分离株为氧化酶和过氧化氢酶阳性，API 20 NE生化鉴定为人苍白杆菌（O. anthropi）（概率为99.9%）（表 1）。命名为JXLH03B。

3.2. 测序和系统发育分析

JXLH03B株16S rRNA基因的序列与苍白杆菌属（Ochrobactrum sp.，GenBank登录号：MH201346.1）、O. anthropi（GenBank登录号：LT671861.1）和羽扇豆苍白杆菌（O. lupini, GenBank登录号：KU217325.1）的序列完全相同。recA 基因经BLAST分析，结果显示与O. anthropi（GenBank登录号：LT671861.1）序列完全相同。

基于16S rRNA基因进行了系统发育分析。如图3所示，JXLH03B株与O. anthropi LMG7991、O. anthropi OAB、O. anthropi LMG5140、O. anthropi LMG3333、O. anthropi LMG3333T、O. anthropi DSM14396、O. anthropi ATCC49188、O. anthropi 7b2C和O. lupini LMG1821处于一个分支。另外，基于recA 基因，JXLH03B株与O. anthropi DSM14396、O. anthropi LMG3333和O. anthropi 10CS0094处于一个分支（图4）。

3.3. 基因组测序和 BLAST

分离菌株基因组测序结果为有两个序列，总长度为4 725 913 bp。G + C含量为56.01%。序列1和序列2的长度分别为2 641 072 bp和2 088 481 bp。BLAST结果表明，分离株的基因组序列与已发表的两株O. anthropi基因组序列之间的相似度为97.54%~98.08%（表2）。

3.4. 小鼠感染试验

JXLH03B株感染BALB/c小鼠的剂量为每只小鼠1×109 ~4×109 CFU。小鼠死亡率为0/5~5/5（表3）。对所有死亡小鼠的大脑取样进行组织病理学检查。所有感染后死亡小鼠的脑组织都有炎症。组织病理学结果显示大脑中有多个坏死灶。脑实质液化，大量嗜中性粒细胞坏死聚集形成结节（如图5中箭头所示）。一些神经胶质细胞崩解（图5）。

3.5. 药敏试验

药敏试验根据动物细菌纸片和稀释法药敏试验判定标准的指导原则，使用纸片扩散试验进行[25]。JX-LH03B株对硫酸庆大霉素、丁胺卡那霉素、恩诺沙星、环丙沙星、多西环素和氟苯尼考敏感，但对青霉素钠、头孢噻呋、卡那霉素硫酸盐、林可霉素和泰乐菌素耐药（表4）。

4. 讨论

O. anthropi 是一种需氧、非发酵、运动、氧化酶阳性、脲酶阳性的革兰氏阴性杆菌，曾被称为“无色杆菌Vd群”[1]。生化反应图谱（API 20 NE）将JXLH03B株鉴定为O. anthropi。Ochrobactrum 属属于布鲁氏菌科，与布鲁氏菌有着较近的系统发育关系[26]。Ochrobactrum 属包含约16个种[27]。16S rRNA基因分析用于鉴定和区分O. anthropi 和布鲁氏菌属[2,26,27]。但是，这种方法（尤其是部分测序）由于其高度的序列相似性而容易误判这些病原体[27]。同时，recA 基因分析提供了更准确的识别和区分[18,27]。我们对JXLH03B分离株进行16S RNA基因测序比对，16S RNA基因比对结果不能完全将该菌株鉴定为O. anthropi，结合recA 基因测序比对的结果将分离株鉴定为O. anthropi 。

对小鼠感染试验表明，该菌株具有一定的致病性，可导致小鼠脑部病变。为了测试JXLH03B株对猪的致病性，分别对9头80日龄的健康猪进行攻毒，分别以每头 猪1.2×109 CFU、1.2×1010 CFU和1.2×1011 CFU的剂量，通过气管注射攻毒。但是，攻毒后没有猪只表现出临床症状，并且攻毒猪的脑样本的组织病理学检查未发现异常（数据未显示）。因此，未能建立O. anthropi JXLH03B株对健康猪的感染模型。在人类中，O. anthropi 主要感染免疫功能低下的患者[ 1,3,4 ]。我们怀疑感染O. anthropi的猪可能是因其他病原感染导致免疫功能受损引起的，因在患病的猪中检测到猪繁殖与呼吸综合征病毒（PRRSV）（数据未显示），而PRRSV会引起免疫抑制性呼吸系统疾病[28]。O. anthropi 广泛分布于土壤和水中[2]，因此，我们怀疑这只猪由于免疫力低下而通过以上途径感染了人苍白杆菌。

人苍白杆菌对除碳青霉烯类以外的几乎所有β -内酰胺类抗生素耐药，原因是其能产生AmpC β -内酰胺酶[29]。我们的研究结果表明，分离株对青霉素和头孢噻呋具有抗性，与文献报道一致[30]。

5. 结论

在本研究中，我们首次从猪体内分离并鉴定了人苍白杆菌（O. anthropi）。这种细菌能够通过试验感染小鼠并导致典型的脑膜炎，这表明O. anthropi 可能具有更广泛的宿主感染谱。
