减少抗微生物药物在动植物性食品生产链中的不当使用是抗微生物药物耐药性全球领导小组（GLG）的一项关键优先级事项[1]。尽管获得高质量的抗微生物药物对动植物的生长很重要，但GLG领导人认识到，如果不减少人用抗微生物药物在动植物性食品生产链中的使用，就不可能预防微生物耐药性（AMR）存在的威胁[1]。在动植物性食品生产链中使用抗微生物药物是一种常见的做法，预测表明从2017年到2030年抗微生物药物的使用量将增加11.5% [2]。现已明确表明在动植物性食品生产链中使用抗微生物药物可导致所使用的药物产生耐药性，至少在农业部门是这样，例如，在中国动物源和人源肠杆菌科细菌中首次检测到质粒介导的多黏菌素耐药性（MCR-1）[3]。自上述研究发表以来，已在100多个国家发现了可转移多黏菌素耐药基因（mcr），其中，许多国家是在农业/食品部门首次发现这种耐药性的。因此，可转移多黏菌素耐药性在国际上的出现不断挑战我们在农业中如何使用所谓的“人用”抗微生物药物（即在人类医学中很重要的抗微生物药物）的观念。

GLG呼吁所有国家完全停止将医学上重要的抗微生物药物用于动植物促生长，通过严格的监管来限制抗微生物药物的预防和治疗使用，消除抗微生物药物的使用以弥补农业实践的不足，并显著减少抗微生物药物的使用，特别是被列在最优先级的至关重要清单上的抗微生物药物[1,4]。然而，虽然这些概念在高收入国家（HIC）可能是可以接受的，甚至是可实施的，但在中低收入国家（LMIC），这些概念的实施将导致肉类价格上涨，并不可避免地使人们因收入减少而加剧贫困。近年来，中国在对抗AMR方面走在了世界前列，自2016年起对多黏菌素在农业中的广泛使用实施了越来越严格的限制，并于2020年发布了一项全面禁令，禁止将所有抗微生物药物用作动物生长促进剂[5]。然而，无论在高收入国家还是低收入和中等收入国家，都面临一个问题：预防和治疗的临界点在哪里？也就是说，在一个动物群体中到底有多少动物患病了才能开始治疗？出于经济原因，农民很可能将这一门槛定得很低，而且可以肯定的是，为确保家庭收入，其将使任何抗击全球AMR的理念变得没有意义。

在2015年发现MCR-1后，中国在2017年禁止使用黏菌素作为促生长剂[6]，并在此后的几年里继续收紧对动植物性食品生产链中使用抗微生物药物的监管[5]。2019年，中国农业农村部出台一项新规定，其目的有三个：禁止所有促生长药物添加至饲料（中药除外）；更新质量标准，使抗微生物药物仅用于预防或治疗；批准新的抗微生物药物仅用于兽医使用[2,7]。欧盟将于2022年1月生效的法规将禁止在农业中使用常规的抗微生物药物[8]。在英国，2013—2017年期间，抗微生物药物在动物卫生部门的使用量下降了40%，政府最近承诺，在未来所有的独立贸易协定中，将把对抗AMR的承诺纳入公布的谈判目标中[9]。

这些措施应该受到赞扬，并且应该鼓励其他国家实施GLG关于动植物性食品生产链中抗微生物药物使用声明中的建议[1]。然而，目前还缺乏配套的基础设施和国际跨学科行动，以确保这些行动在全世界各地实现其预期目标。此外，如上所述，必须有地方和国家的财政支持，在不造成贫困的情况下鼓励这种激励措施。我们并不完全了解这些政策在欧洲和中国对动物生长和动植物性食品生产链效率的长期影响。这些举措是前所未有的，监测其影响对于理解如何在世界范围内（尤其是中低收入国家）实施类似政策至关重要。虽然这些干预措施值得关注和重视，但用于动物饲料（以及用于预防和治疗）的抗微生物药物的国际贸易仍有增无减，许多中低收入国家仍然接受多黏菌素等抗微生物药物，这在2022年似乎是荒谬的。因此，早就应该对抗微生物药物销售进行全球管理——尤其应严格监管兽用抗微生物药物，其使用量已远远超过人用抗微生物药物使用量。

预计到2030年，全球肉类消费量将增长14% [10]，尤其是中低收入国家需要继续确保高效的养殖方式，以满足不断增长的人口需求。中低收入国家的农民需要得到支持，以便在农场卫生、管理和畜牧业方面做出必要的改进，从而限制抗微生物药物的使用，同时仍然能保护动物生长，避免牲畜因疾病遭受灾难性损失。目前仍然缺乏相应政策来支持实现上述改变，仍然需要时间动员全球行动，同时还需要必要的财政支持。

要求中低收入国家的农民减少抗微生物药物的使用将是一个挑战，因为没有类似疗效的药物可以替代它们。在减少使用的同时，我们还需要找到可替代的抗微生物药物，既保护动物生长和高效农业，又不会与对人类至关重要的抗微生物药物产生交叉耐药性。与人用抗微生物药物的要求相比，此类替代抗微生物药物的一些要求可以很容易满足（如毒性）；然而，其他方面，如环境半衰期、成本（例如，多黏菌素目前的成本约为10美元·kg^-1），以及易合成和生产则不太容易实现。为了解决这些问题，英力士牛津研究所（IOI）提出了确定仅用于动物的新型抗微生物药物、治疗方法和组合等重要理念，包括建立标识“无人用抗微生物药物”的食品标签（NHA）[11]。
