Ventricular tachycardia in a disseminated MDR-TB patient: a case report and brief review of literature

Although significant breakthroughs have been achieved in tuberculosis management, we still encounter numerous difficulties in diagnosis and treatment of the disease. Additionally, a new challenge, multidrug-resistant tuberculosis (MDR-TB) with unspecific clinical presentation, often results in delayed diagnosis. In this paper, we reported a case of disseminated tuberculosis with rare presentation of ventricular fibrillation, which proved resistant to both isoniazid and rifampicin. A review of literature showed that ventricular fibrillation or tachycardia in tuberculosis patients with pericarditis or myocarditis has been sporadically reported in the past, but none has been conducted involving patients with MDR-TB infections.

bright luminescent hierarchical materials from a tripodal benzoate antenna and heptadentate Eu(III) and Tb

The europium heptadentate coordinatively unsaturated (Eu(III)) and the terbium (Tb(III)) 1,4,7,10-tetraazacyclododecane (cyclen) complexes 1 and 2 were used in conjunction with ligand 3 (1,3,5-benzene-trisethynylbenzoate) to form the supramolecular self-assembly structures 4 and 5; this being investigated in both the solid and the solution state. The resulting self-assemblies gave rise to metal centered emission (both in the solid and solution) upon excitation of 3, confirming its role as a sensitizing antenna. Drop-cased examples of ligand 3, and the solid forms of 4 and 5, formed from both organic and mixture of organic-aqueous solutions, were analyzed using Scanning Electron Microscopy, which showed significant changes in morphology; the ligand giving rise to one dimensional structures, while both 4 and 5 formed amorphous materials that were highly dense solid networks containing nanoporous features. The surface area (216 and 119 m2·g1 for 4 and 5 respectively) and the ability of these porous materials to capture and store gases such as N2 investigated at 77 K. The self-assembly formation was also investigated in diluted solution by monitoring the various photophysical properties of 3–5. This demonstrated that the most stable structures were that consisting of a single antennae 3 and three complexes of 1 or 2 (e.g., 4 and 5) in solution. By monitoring the excited state lifetimes of the Eu(III) and Tb(III) ions in H2O and D2O respectively, we showed that their hydration states (the q-value) changed from ~2 to 0, upon formation of the assemblies, indicating that the three benzoates of 3 coordinated directly to the each of the three lanthanide centers. Finally we demonstrate that this hierarchically porous materials can be used for the sensing of organic solvents as the emission is highly depended on the solvent environment; the lanthanide emission being quenched in the presence of acetonitrile and THF, but greatly enhanced in the presence of methanol.

Spectrokinetics study of probable effects of diverse inorganic ions on bleaching of dye

摘要: Toluidine blue (TB) is an important anticoagulant metachromasia molecule showing a pronounced variation in the visible spectrum due to the aggregation phenomenon and electrostatic interaction with the charged synthetic and biologic polymers. The current study describes the interactive role of diverse inorganic material ions on the bleaching of toluidine blue (tolonium chloride) (TB ) with urea in acidic and basic media using the spectrophotometric technique. The spectra of TB and urea with different cations and anions were monitored and their characteristic features are presented here. The negative effect of added cations on reduction may be the result of altered electron pathways which led to suppression of the reduction/bleaching of TB, while a slight decrease in dye reduction by added anions may be due to the scavenging of the OH* radical. It has been observed in the case of Co that in addition to the electron-transfer reaction, other processes like layer and precipitate formation also appear to be taking place. The dye bleaching process followed pseudo first order kinetics with respect to TB, urea, and H ion, whereas significant decoloration in the presence of urea proved that reductants control the redox reaction. No decoloration in acidic medium with diverse ions was seen compared to alkaline media, showing that water pH played an important role in the bleaching of dye. The reduction/bleaching of dye was investigated at different temperatures, and energy parameters were evaluated for a TB -Urea reaction, including the energy of activation ( = 39.60 kJ·mol ), enthalpy of activation (? = 34?kJ·mol ), entropy of activation (? = 146.5 kJ mol ·K ), and free energy of activation (ΔG* = -52.35 kJ·mol ). A mechanism of interaction of diverse ions in dye bleaching and a mechanism of reduction based on the above findings is proposed.

Improvement of sludge dewaterability with modified cinder via affecting EPS

摘要: The relationship between the improvement of sludge dewaterability and variation of organic matters has been studied in the process of sludge pre-conditioning with modified cinder, especially for extracellular polymeric substances (EPS) in the sludge. During the conditioning process, the decreases of total organic carbon (TOC) and soluble chemical oxygen demand (SCOD) were obviously in the supernatant especially for the acid modified cinder (ACMC), which could be attributed to the processes of adsorption and sweeping. The reduction of polysaccharide and protein in supernatant indicated that ACMC might adsorb EPS so that the tightly bound EPS (TB-EPS) decreased in sludge. In the case of ACMC addition with 24 g·L , SRF of the sludge decreased from 7.85 × 1012 m·kg to 2.06 × 1012 m·kg , and the filter cake moisture decreased from 85% to 60%. The reconstruction of “floc mass” was confirmed as the main sludge conditioning mechanism. ACMC promoted the dewatering performance through the charge neutralization and adsorption bridging with the negative EPS, and provided firm and dense structure for sludge floc as skeleton builder. The passages for water quick transmitting were built to avoid collapsing during the high-pressure process.

Study on factors affecting TB/HIV co-infection in four counties of China

摘要: The factors affecting case detection of tuberculosis (TB)/human immunodeficiency virus (HIV) co-infection cases were explored in order to provide evidence-based formulation of a TB/HIV co-infection control strategy in China. Four counties with different modes of HIV transmission and varying cooperation mechanisms between TB and HIV institutions were selected. HIV test among TB patients and TB examination among people living with HIV/acquired immunodeficiency syndrome (AIDS) were conducted. Patients also completed questionnaires on potential factors affecting TB/HIV co-infection. The results showed that 19 (1.7%) were HIV-positive among 1089 cases of people living with active TB who received HIV testing. Conversely, there were 126 (10%) cases of active TB among 1255 cases of people living with HIV/AIDS. Among the newly discovered 145 cases of TB/HIV co-infection, AIDS institutions found 86.9% of these co-infections and 14.1% came from TB institutes. In different registration categories, there were 31 (21.4%) cases of smear positive TB, 104 (71.7%) cases of smear negative TB, and extrapulmonary cases accounted for 10 (6.9%). Gender, occupation, marital status, ethnic group, education, HIV transmission route, CD4 level, and type of TB susceptible symptoms had significant effects on whether HIV/AIDS patients were co-infected with TB disease (<0.01). Sex, age, ethnic group, occupation, and type of tuberculosis had no significant influence on whether TB patients had HIV infection. AIDS institutions could find more TB/HIV co-infection patients in comparison with TB institutions, particularly in patients with smear negative pulmonary tuberculosis and extrapulmonary tuberculosis. Therefore, a cooperation mechanism should be built up between TB and HIV control systems, and routine TB checks should be conducted for HIV/AIDS patients. In areas where sex or drug use is the major transmission mode of HIV infection, antiretroviral therapy should be expanded, treatment adherence should be improved and patients’ immunity level should be increased in order to decrease TB incidence. Cough and expectoration of more than two weeks, bloody sputum, and recurrent fevers could be revealing symptoms in TB screening of HIV/AIDS patients.
